Your First Appointment
Please read our Welcome letter. It will provide a brief introduction to our clinic, what you can expect from us, and what we ask of you. You may find answers to additional common questions on our FAQ page. Also, please feel free to call (570) 409 - 9233 to ask additional questions.
Our health history questionnaire, informed consent, and cancellation policy, must all be completed for your first appointment. You may print them and bring them completed to your first appointment. If you wish to complete them at the office, we ask you to come at least 10 minutes PRIOR to your scheduled appointment time to complete them.
